Before You Submit

To receive credits for EIN 4944 or ESI 4949, students who participated or are currently participating in internships must submit this form, one for each internship or co-op position.

After this form is submitted, advising staff will send your employer / supervisor an e-mail requesting an evaluation of your work performance. Contact and encourage your supervisor/employer to complete his/her evaluation as soon as possible. If no employer evaluation is submitted on your behalf before the last day of class, you will get an incomplete.

For each internship or co-op lasting at least 200 hours, you can obtain one credit hour for EIN 4944 or ESI 4949 (both of which can be repeated up to 3 credit hours). To obtain the credit for internship, you need to register for EIN4944 either in the current or future semester. For co-op, you need to register for ESI 4949. You need to check with the undergraduate advisor.
